Since an over-the-counter formulation of orlistat is now widely available, it is important to ask patients if they are using orlistat (Alli) and might be at risk for fat-soluble nutrient deficiency. Roche, the pharmaceutical company that produces orlistat, recommends that patients using orlistat take a daily multivitamin supplement with the important fat-soluble vitamins at least two hours before taking orlistat. Furthermore, patients with malabsorption syndromes should not take orlistat because the drug will further aggravate gastrointestinal symptoms and nutrient deficiencies.

These nutritional deficiencies are related to both poor dietary intake and malabsorption [125]. Fat-soluble nutrient deficiency is more likely for procedures that include biliopancreatic diversion due to impaired micelle formation and lipase activity. A 1982 study found that carotenoid levels dropped rapidly and remained low in patients after jejunoileal bypass surgery [126]. To date, carotenoid level changes have not been studied for the now, more common Roux-en-Y gastric bypass. However, it is felt that overall malabsorption is less common for Roux-en-Y gastric bypass than previous procedures that diverted crucial absorptive gastrointestinal mucosa and the biliary and pancreatic digestive apparatus [127].
